New Lyminge Surgery Annual Health Reviews
From the 1st April 2025, New Lyminge Surgery will be introducing a new system for annual health reviews which means patients will be invited for a review around their month of birth.
During this transition period, you may find that you have your annual review slightly earlier or later than normal – this means that you may be invited for a review even if you have recently had one. Please be aware that if you do not wish to attend another review, you will not be invited for your next annual health review until your month of birth the following year.
This system should mean that any tests and checks needed to manage your long term condition can be done at the same time, resulting in having to attend fewer appointments.
What is an annual review?
We provide patients with long-term conditions an annual health review so that your health can be monitored regularly. These reviews are in addition to any on-going appointments or tests you may already be having with your doctor.
The conditions for which you will be invited for an annual health review include:
- Diabetes, including those with pre-diabetes
- Hypertension (high blood pressure)
- Heart disease
- Stroke
- Atrial fibrillation
- Chronic kidney disease (CKD)
- Long term mental health problems
- Dementia
- Asthma
- Chronic obstructive pulmonary disease (COPD)
- Learning disabilities
When will I be invited for an annual review?
You should expect an invitation for an annual review around your month of birth. This should make it easier for patients to remember when their annual review is due.
Some patients may also need interim reviews throughout the year.
How will I be invited for my review?
Your invitation will be in the form of a text message, if we have your mobile number recorded but if not, you will receive a letter in the post. Please ensure we have the most up to date contact information for you.
The text message we send you will include a link to allow you to book an appointment with a Health Care Assistant (HCA)/Nurse online.
We sometimes send patients an online questionnaire so that you can provide us with any relevant information relating to your health condition. This will be reviewed by a clinician who will contact you if we need any further information or if we need to book you an appointment.
What appointments do I need to book?
The appointments you require will depend on the long-term condition you have. Some reviews will just be an appointment with a Health Care Assistant (HCA)/Nurse who will do a blood test, foot check (for diabetic patients only), as well as checks on height, weight and blood pressure.
For some long-term conditions, you may also need a follow up appointment with a nurse or GP to complete your review. Please ask your HCA/Nurse if a follow up appointment is required so they can book this for you, or alternatively, this can be done by our reception team.
